• Record, photograph, and otherwise capture my likeness, image, voice, and/or performance.

• Use, reproduce, distribute, and display such recordings in social media videos, promotional content, advertisements, and other marketing materials related to Office Hours with Kris Johnson and Kris Johnson Music, LLC.

• Edit, adapt, or incorporate the recordings into other works at the company's discretion.

• Publish and broadcast the content across any platform, including but not limited to Instagram, Facebook, YouTube, TikTok, and paid advertising channels.

• I am not required to complete this form, and my decision will not affect my participation in the event or session.

• If I do not submit this form, Kris Johnson Music, LLC will take reasonable steps to ensure my image does not appear in any published video, though any audio of me playing may still be included.

• This authorization is perpetual and covers current and future media formats.

• I release Kris Johnson Music, LLC from any claims arising from the use of the recorded content as described above.
